Support for _definst_ in nginx-rtmp-module

With current master it's a little easier to stream from Wowza/FMS. Now nginx-rtmp handles _definst_ in application names. The following lines are equivalent

ffplay rtmp://localhost/myapp/_definst_/mystream
ffplay rtmp://localhost/myapp/mystream
